What are the responsibilities and job description for the Nurse Practitioner - Home Setting position at High Care Palliative?
We are seeking a Nurse Practitioner to perform palliative evaluations in the home setting. Palliative aims to support chronically ill patients in our community. They direct and advise the patient care plan and work to liaise between the office admin support team and the patient's Insurance provider to give quality care and overall satisfaction. Looking for a compassionate and hardworking individual for seeing and visiting patients in their home in the community.
QUALIFICATIONS:
Credentials: Must have a current Registered Nurse license, a current unrestricted Nurse Practitioner license issued by the State, a current Drug Enforcement Agency (DEA) Certificate. Current Basic Life Support license (BLS).
Experience: Knowledge or experience in Hospice, Palliative, or Home health, or similar field with symptom or medication management, assessment and plan of care adherence.
Responsibilities:
- Care for ill, injured, or convalescing patients
- Provide basic patient care and treatment
- Collaborate with registered nurses to administer prescribed medications
- Educate and update patients on medical treatments
Qualifications:
- Participating as a member of the interdisciplinary group that establishes and/or updates the individual's plan of care, when serving as the attending physician. Advising and reporting activities and developments that relate to the medical care, of patients.
- Promoting the program, including developing and maintaining effective and positive relationships with health care professionals within the community. Previous experience in nursing or other medical field
- Remaining current in the techniques of palliative medicine, through continuing medical education, medical journals, clinical research, and peer contact.
- Participating in utilization review and quality assurance activities, including chart review and audits, and resolving physician’s assistant conflicts.

- Excellent written and verbal communication skills
PHYSICAL REQUIREMENTS:
1. Evidence of good physical and mental health.
2. Able to lift 40 lbs. from floor to waist and waist to shoulder.
3. Able to lift 25 lbs. from shoulder to overhead.
4. Able to perform patient transfers, utilizing appropriate techniques including assistance.
5. Average vision/hearing
6. Frequent bending and reaching
7. Repetitive standing, walking and use of hands.
9. Able to drive a car a minimum of 4 hours/day
Salary : $150,000 - $160,000
