Case Manager – Hickory Behavioral at Cambridge

About the Role: Under the immediate supervision of the Clinical Manager, the Case Manager is responsible for each component of the client’s treatment planning, client’s behavioral health plan, including all programmatic implementation.
Case managers will coordinate aftercare plans such as next level of care, MAT service appointments, mental health continuums, primary physician appointments, relapse prevention support within the community including NA/AA meetings and identify transportation for client at time of discharge.
The Case Managers will also meet with their assigned case load to adhere to documentation requirements through Medicaid standards.

Job Qualifications:
- Bachelor’s Degree Preferred in a Behavioral Health discipline or related field
- Demonstration of a client-centered team approach, with advanced computer skills.
- Understanding of Substance Use Disorders and Dual Diagnosis
- Must maintain a positive demeanor and clear understanding of professional boundaries with clients and staff.
- Ability to follow directives explicitly, adhere to facility policies, and exercise time management skills.
- Completion of CPR and HWC prior to working the floor
- Schedule flexibility may be required to accommodate patients
- Must hold a valid driver’s license with reliable transportation.
Key Responsibilities:
- Participates in performance improvement activities
- Collaborates with primary therapist on client treatment goals and discharge planning
- Contacts assigned clients within 24-hours of admission and thereafter
- Completes Behavioral Health Plan, My SNAP, and BAM within 24-hours of admission
- Documents Case Management Note and updates Behavioral Health Plan and BAM in the EMR for assigned clients
- Assists assigned clients with coping skills, triggers, and a relapse prevention plan
- Leads groups and documents individual session notes in the EMR
- Administers the client Daily Self-Assessment and documents in EMR daily
- Monitors clients contacts and telephone calls
- Assists assigned clients with court appointments, communication with probation and/or legal representative, personal business, benefits assistance, etc.
- Assists assigned clients with safe and supportive discharge planning including housing, employment, 30-day after care plan calendar, and documents in EMR
- Adheres to HIPAA policies including release of client information
- For clients discharging AMA, case manager completes the BAM, Suicide Risk Assessment, AMA Questionnaire and Refusal of Treatment AMA Form when directed
- Assists BHTs as needed
- Reports safety hazards to supervisor or Executive Director
- Performs appropriate hand hygiene throughout the shift
- Uses personal protective equipment (PPE) appropriately
Compensation: $20-24/hr
Salary : $18 - $21
