As a Licensed Practical Nurse (LPN) at Hi-Hope Service Center, you will support client health and wellness by performing nursing tasks, medication administration, care coordination, and clinical documentation under Georgia Medicaid Waiver guidelines.
- Complete assigned nursing care tasks and maintain timely, accurate documentation
- Review and manage CareSuite QuickMAR, Therap, and paper MARs
- Assist with medication administration and health care planning
- Conduct basic triage and first aid when needed
- Transport residents to medical appointments as necessary
- Ensure medical follow-ups and health risk tracking for assigned individuals
- Participate in staff meetings and collaborative care planning
Requirements
- Active LPN license in Georgia
- Minimum 1–2 years of experience in a clinical, residential, or ID/DD setting preferred
- Proficient in Microsoft Office, EMAR systems, and electronic documentation
- Must be 21 , pass a physical exam, drug screen, background check, and TB screening
- Must have a valid Georgia driver's license and reliable transportation
