Licensed Practical Nurse (LPN)

As a Licensed Practical Nurse (LPN) at Hi-Hope Service Center, you will support client health and wellness by performing nursing tasks, medication administration, care coordination, and clinical documentation under Georgia Medicaid Waiver guidelines. 

1. Complete assigned nursing care tasks and maintain timely, accurate documentation
    
2. Review and manage CareSuite QuickMAR, Therap, and paper MARs
    
3. Assist with medication administration and health care planning
    
4. Conduct basic triage and first aid when needed
    
5. Transport residents to medical appointments as necessary
    
6. Ensure medical follow-ups and health risk tracking for assigned individuals
    
7. Participate in staff meetings and collaborative care planning

Requirements

1. Active LPN license in Georgia
    
2. Minimum 1–2 years of experience in a clinical, residential, or ID/DD setting preferred
    
3. Proficient in Microsoft Office, EMAR systems, and electronic documentation
    
4. Must be 21 , pass a physical exam, drug screen, background check, and TB screening
    
5. Must have a valid Georgia driver's license and reliable transportation
