Description
The Community Access Individual provides person-centered support, guidance, and skill-building to help individuals successfully navigate community life, develop relationships, and participate in meaningful activities. Requirements
Key Responsibilities
- Provide respectful, person-centered support aligned with Individual Service Plans (ISP).
- Facilitate participation in community activities, volunteer opportunities, and social events.
- Teach and coach daily living, socialization, and independent community navigation skills.
- Serve as a positive role model in behavior, communication, and professionalism.
- Maintain accurate and timely documentation of services and individual progress.
- Ensure health, safety, and well-being by following all safety and emergency protocols.
- Collaborate with team members, families, and community partners to support positive outcomes.
- Participate in required trainings, staff meetings, and professional development.
Qualifications
- High school diploma or GED required; additional relevant training preferred.
- Experience supporting individuals with intellectual/developmental disabilities, vocational counseling, job coaching, or education/training preferred.
- Strong communication and interpersonal skills.
- Basic computer proficiency for documentation and communication.
- Valid Georgia driver’s license required.
- CPR/AED and First Aid certification (or ability to obtain within 90 days).
- Ability to complete Transportation & Vehicle Emergency Evacuation and De-escalation (QBS) training within 90 days.
- Must be 21 years or older and able to pass:
- Pre-employment physical and TB screening
- Drug screening
- Motor Vehicle Record (MVR) check
- Criminal background check
Physical Requirements
- Frequent standing, walking, kneeling, and lifting up to 50 lbs.
- Driving individuals to and from community activities.
- Work performed in indoor and outdoor environments.
- 80–100% community travel may be required.
