Multiple positions

All Shifts

$19.24/hour - $30.41/hour

2nd Shift Differential $4.00 per hour, 3rd Shift Differential $7.00 per hour

Weekend $2.00 per hour

Salary does not reflect $520.00 direct care pay (to be paid at $20 bi-weekly)

The State of New Hampshire, Department of Health & Human Services, Healthcare Facilities, Glencliff Home have full-time vacancies for Licensed Nursing Assistants I-II.

Summary:

LNA I

Under the direct supervision of licensed nursing staff, provides essential support and direct care to residents by assisting with daily living activities, monitoring health conditions, and ensuring a safe, clean, comfortable, and enriching environment.

LNA II

With direction and oversight by licensed nursing staff, independently provides essential support and direct care to residents by assisting with daily living activities, monitoring health conditions, and ensuring a safe, clean, comfortable, and enriching environment. Mentors new nursing assistants.

YOUR EXPERIENCE COUNTS: Each additional year of approved formal education may be substituted for one year of required work experience and/or each additional year of approved work experience may be substituted for one year of required formal education. 

LNA I

MINIMUM QUALIFICATIONS:

Education/Experience: High school diploma or equivalent credential

License/Certification: 

1. NH Nursing Assistant license

2. Driver’s license

Other Requirements:

1. May be required to work non-traditional work hours to meet the needs of the facility

2. Must possess physical stamina for prolonged standing and to perform tasks such as lifting, transferring, and transporting residents and materials

Trainee Option: Candidates without a high school diploma or equivalent credential, or without a NH Nursing Assistant license, may be considered and hired in 'trainee status' for this position. Individuals hired in ‘trainee status’ will have their responsibilities adjusted as necessary for compliance with facility credentialling.

PREFERRED QUALIFICATIONS: Experience in long-term or geriatric care setting or other role involved in the provision of medical care. Strong verbal and written communication and interpersonal skills. CPR and First Aid certification.

AFTER-HIRE REQUIREMENTS: If hired in trainee status, must obtain a NH Nursing Assistant license within 6 months of hire. Candidates may be able to obtain their license through participation in onsite training program.

For promotion from the 31-1130 Nursing Assistants, Orderlies, and Psychiatric Aides-1 job title to the 31-1130 Nursing Assistants, Orderlies, and Psychiatric Aides-2 job title as a Residential LNA, the eligibility requirements in Personnel Rule 303.08 Broad Group Level Advancement must be satisfied, as well as the following criteria:

1. Has one year of experience functioning as a Licensed Nursing Assistant or other practicing medical professional

2. Demonstrates sufficient competency in performance of required tasks to serve as mentor for new nursing assistants

3. Receives a satisfactory performance evaluation within the past 12 months

4. Is up-to-date on all mandatory in-house training

5. Is recommended for promotion by supervisor(s) and approved by the director of nursing

LNA II

MINIMUM QUALIFICATIONS:

Education/Experience: Equivalent combination of 1 year education and experience after completion of high school

License/Certification:

1. NH Nursing Assistant license

2. Driver’s license

Other Requirements:

1. May be required to work non-traditional work hours to meet the needs of the facility

2. Must possess physical stamina for prolonged standing and to perform tasks such as lifting, transferring, and transporting residents and materials

Trainee Option: Not applicable

PREFERRED QUALIFICATIONS: Strong preference for candidates with at least one year of experience as a practicing Licensed Nursing Assistant or other practicing medical professional. Experience in a long-term or geriatric care setting. Strong verbal and written communication and interpersonal skills. CPR and First Aid certification.

Criteria: 31113002IBA01 (Medication Certification)

For the in-band advancement to be applied, the eligibility requirements in Personnel Rule 303.09 In-Band Advancement must be satisfied, as well as the following criteria:

1. Holds a valid NH Medication Nursing Assistant license

2. Assumes responsibility for administering medications

Broad Group Level Advancement Available: ☒ Yes ☐ No

Criteria: 31113002BGA01 (Level 2 to Level 3)

For promotion from the 31-1130 Nursing Assistants, Orderlies, and Psychiatric Aides-2 job title to the 31-1130 Nursing Assistants, Orderlies, and Psychiatric Aides-3 job title as a Residential LNA, the eligibility requirements in Personnel Rule 303.08 Broad Group Level Advancement must be satisfied, as well as the following criteria:

1. Holds five NH Nursing Board Approved competencies

2. Receives a satisfactory performance evaluation within the past 12 months

3. Is up-to-date on all mandatory in-house training

4. Is recommended for promotion by supervisor(s) and approved by the director of nursing

Responsibilities:
LNA I

DETAILED TASKS (STANDARD):

Assists residents with activities of daily living (ADLs), including getting out of bed, bathing, dressing, grooming, toileting, standing, walking, or exercising. Ensures proper dietary intake and adequate hydration and that dietary restrictions and preferences are observed. Supports mobility by helping residents move between beds and wheelchairs, and transferring residents to special functions, trips, appointments, and other activities within and outside the facility.

Utilizes appropriate equipment and follows established safety protocols to ensure resident’s comfort and dignity. Adapts techniques and frequency based on individual needs and preferences and established care plans. Promotes independence in meeting individual needs.

Acts in accordance with established care plans; participates in Plan of Care meetings as assigned/needed. Documents care provided and individual’s behavior/response; maintains accurate records. Monitors and records vital signs such as temperature, blood pressure, and pulse, as well as food/liquid intake and urinary/fecal output. Collects specimens for routine laboratory testing. Reports changes in residents’ physical, mental, or emotional condition to a supervisor or more senior nursing staff in a timely manner.

Replenishes charts and retrieves, stocks, and distributes medical supplies and personal protective equipment (PPE) as required. Reports any shortages.

Follows infection control protocols and safety procedures. Maintains cleanliness and hygiene in resident rooms and common areas. Performs daily assigned tasks such as clothing distribution, emptying laundry hampers, and cleaning furniture.

Prepares residents for appointments and medical team consults. Provides a level of security/support for the resident.

Demonstrates empathy and patience in providing care and assistance. Encourages and maintains open communication and positive interactions with residents, families, guardians, and other visitors.

Attends and participates in in-service educational programs as well as organization meetings and committees. Responds to supervision and works cooperatively with others in provision of care and operation of facility.

DETAILED TASKS (AGENCY/POSITION-SPECIFIC):

May operate a facility vehicle to deliver specimens off-site and/or transport residents to appointments and other activities as needed.

Implements safety protocols during interactions, including restraint procedures when necessary.

May accompany residents to recreational activities, recreational outings, and appointments. Provides emotional support and companionship to residents.

LNA II

DETAILED TASKS (STANDARD):

Exercises some independent judgement in providing daily care and resolving minor physical and emotional needs. Assists residents with activities of daily living (ADLs), including getting out of bed, bathing, dressing, grooming, toileting, standing, walking, or exercising. Ensures proper dietary intake and adequate hydration and that dietary restrictions and preferences are observed. Supports mobility by helping residents move between beds and wheelchairs, and transferring residents to special functions, trips, appointments, and other activities within and outside the facility. Communicates resident needs to others when outside own scope of duties.

Utilizes appropriate equipment and follows established safety protocols to ensure resident comfort and dignity. Adapts techniques and frequency based on individual needs and preferences and established care plans. Promotes independence in meeting individual needs, while recognizing variations in capabilities and limitations.

Performs treatment procedures within the LNA scope of practice. Follows medical orders and acts in accordance with established care plans. Participates in Plan of Care meetings as assigned/needed; presents patient/resident information when applicable. Coordinates visits from consulting professional health team members, including dentists, podiatrists, and physical therapists. Assists health team members and provides a level of security/support for the resident.

Documents care provided and individual’s behavior/response; maintains accurate records. Monitors and records vital signs such as temperature, blood pressure, and pulse, as well as food/liquid intake and urinary/fecal output. Assists with screening for medical issues and collects specimens for routine laboratory testing. Observes and reports changes in residents’ physical, mental, or emotional condition to a supervisor or more senior nursing staff in a timely manner.

May serve as mentor for new nursing assistants providing guidance on established facility/unit responsibilities, policy and procedures to meet the standards of care.

Replenishes charts and retrieves, stocks, and distributes medical supplies and personal protective equipment (PPE) as required. Maintains proper inventory and prepares/submits supply orders when necessary. Documents equipment/maintenance issues and repairs needed.

Follows infection control protocols and safety procedures. Maintains cleanliness and hygiene in resident rooms and common areas. Performs daily assigned tasks such as clothing distribution, emptying laundry hampers, and cleaning furniture.

Demonstrates empathy and patience in providing care and assistance. Encourages and maintains open communication and positive interactions with residents, families, guardians, and other visitors.
