LVN - LPN- Admissions Nurse- PM Shift- Full-Time

 Our Licensed Vocational Nurses/Licensed Practical Nurses (LVN/LPN) are responsible for the nursing needs of residents, while actively incorporating our “Philosophy for person-directed Care.” You will ensure a safe and healthy environment for residents, staff, and visitors to participate in the resident’s daily life rhythm within the Health Center. You would help develop assignments in coordination with the care partners and keep each team member accountable for resident care and satisfaction in a skilled nursing environment.

Hourly: $32.25- 37/hr depending on experience

Shift: Full-Time, Monday- Friday

2:30PM-10:30PM

To be successful in the role, you would have: 

• Provides resident care, including administering medications and documents each treatment and maintains weekly skin condition reports
• Maintain communication with doctors, laboratory, pharmacy and residents’ families concerning resident care;
• Attend care plan meetings;
• May lead caregivers or direct some caregiving services;
• Conducts admission assessments as well as assessments for change of condition under the direction of the RN

Requirements 

• Current LVN/LPN in the state for which applying
• Current CPR certification
• Prior nursing experience: senior care, skilled nurse, post-acute or sub-acute care preferred
• Prior Admissions experience with PCC background
