Title: Medical Records Coordinator
Status: Full-Time, Non-Exempt
Reports To: Nursing Home Administrator
Pay Range: $21-26 per hour
THE ROLE
You will lead with both heart and discipline. That means keeping every resident’s medical record accurate, complete, and survey‑ready. You will stay ahead of state and federal recordkeeping requirements, ensure timely physician orders and consents, and create a system where clinical staff can easily find and document what they need. Surveyors will see a clean, organized chart – and you will be the reason.
WHAT YOU BRING TO THE FACILITY
- THE FOUNDATION
- High school diploma or equivalent (associate or bachelor’s degree in health information management preferred).
- Certification as a Registered Health Information Technician (RHIT)or Certified Medical Records Technician is a plus.
- No active violations on any facility employment record.
- THE JOURNEY
- Minimum 1 yearof medical records experience in a skilled nursing facility or healthcare setting.
- Proven track record of maintaining organized, deficiency‑free medical records.
- Experience with electronic medical records (EMR) systems and document scanning.
- Familiarity with state and federal record retention and release of information regulations.
- THE CRAFT
- Deep fluency in applicable state and federal medical record requirements(including HIPAA).
- Ability to audit charts for completeness, signatures, and timely orders.
- Strong attention to detail – you catch a missing physician order before a surveyor does.
- Proficient with EMR software, Microsoft Office, and document management systems.
- THE CHARACTER
- Calm, organized presence during a records audit or survey.
- Treats nursing, therapy, and administrative staff as collaborative partners in documentation.
- Handles family requests for records with empathy and sound judgment (following HIPAA).
- Passion for building a culture where every chart is complete, accurate, and survey‑ready.
WHAT YOU WILL BE DOING
- Maintain and organize all resident medical records (paper and electronic) per state and federal regulations.
- Ensure physician orders are signed, dated, and implemented in a timely manner.
- Audit closed and active charts for completeness, including consents, advance directives, and care plans.
- Process requests for medical records from families, physicians, and government agencies (HIPAA compliant).
- Assist with survey preparation by pulling charts and organizing requested documentation.
- Scan and index documents into the EMR system; manage record retention and destruction schedules.
- Support nursing and therapy staff with documentation training and EMR troubleshooting.
