Program Description

The Children's Community Support Collaborative (CCSC) is a program of The Home for Little Wanderers located in a brand new, state of the art facility overlooking the Boston skyline in the Roxbury Crossing neighborhood of Boston. The primary goal of the Collaborative is to support families to remain together or return to being together as soon as clinically possible, by providing wraparound services a family needs to be successful in this goal. We provide intensive community based in-home treatment to help families remain together, rebuild connections that have been broken, and provide a unique group home treatment environment for clients not able to remain in their homes.

What kind of clinical environment do we operate in?  First and foremost, we offer a highly supportive, well resourced, and flexible approach to care.  Weekly supervision, multidisciplinary teams, and intensive clinical training support not only the best possible outcomes for our families, but a work environment where the collective wisdom and approaches of all the supporting disciplines lead to amazing outcomes for kids and families. Our multi-disciplinary team includes occupational therapists, peer specialists, outreach workers, milieu staff, nurses, a psychiatrist, clinicians and administrators, and offers many opportunities for connection, collaboration, growth and a collegial atmosphere.

We are committed to trauma-informed care that centers the voices of youth and families. To this end, we use principles and practices from Integrative Treatment of Complex Trauma (ITCT), Therapeutic Crisis Intervention (TCI), Restorative Practices, and Permanency work to support family reunification and stabilization.  We focus on providing age-appropriate, culturally sensitive therapy for clients and families in a variety of settings both in the community as well as within group home placement. 

How You Will Be Making A Difference

  • Works one-on-one with families and youth and maintains regular frequent contact with the parent(s)/caregiver(s) in order to provide education and support throughout the care planning process
  • Participates as a member of the CPT and clearly outlines the goals of the Family Support and Training Services
  • Works closely with families and any existing/referring behavioral health provider(s) to implement the objectives and goals identified in the Individual Care Plan
  • Teaches parents/caregiver how to promote linkages with other treatment providers and other community resources and work with the Intensive Care Coordinator
  • Educates parents/caregivers about how to effectively navigate the child-serving systems and about the existence of informal/community resources available; and facilitates access to these resources
  • Presents client/family material; consult with supervisor and team members on challenging and difficult client/family issues. Participates in regular individual and group supervision. Attends clinical training and safety practice training.
  • Documents significant family interactions and collateral work as per MassHealth requirements
  • Participates in program activities, including but not limited to, staff and team meetings, training and development needs of the program, and primary on-call rotation
  • May be responsible for training staff from Hyde Park and Park Street CSA’s in High Fidelity Wraparound: Tier 1 Certification

Experience

  • Must be a caregiver who has experience raising a child or youth with special needs, preferably with significant mental health needs
  • May or may not have program specific experience
  • Excellent cultural awareness and sensitivity
  • Computer literate, including Microsoft Office and Internet Explorer, with the ability to learn new software applications
  • May require knowledge of a second language such as Spanish or Haitian Creole

Driving Requirements

  • May be required to operate a vehicle 
  • May be required to have a valid driver’s license and a “satisfactory” driving record
  • May be required to have access to a personal vehicle for use during work hours
  • May be required to use personal vehicle to transport clients on occasion if/when needed
  • If required to transport clients, must maintain the minimum amounts of liability insurance as required by The Home and/or other regulatory requirements and vehicle registration must be current

Pay Range Positioning Summary

The pay range listed represents the company’s good faith estimate of the salary or wage range for this position at the time of posting, as required under applicable law. Actual compensation will be based on a variety of factors, including a candidate’s relevant experience, education, skills, and internal equity relative to other employees in similar roles. Candidates with less directly related experience may be placed toward the lower end of the range, while those with extensive or specialized experience may be placed toward the higher end. Market conditions and budget considerations may also influence final pay decisions.
