What are the responsibilities and job description for the Registered Nurse position at HESSCO Elder Services?
HESSCO, a non-profit social services agency, has a full-time (35 hours per week) position available for a Registered Nurse. This position does not require you to work nights, weekends or holidays.
The RN participates in service planning and nursing assessments of consumers to determine their medical eligibility for programs funded by the Executive Office of Aging and Independence. The nurse performs in-home nursing assessments to determine the personal care needs of Home Care consumers and nursing facility residents. The RN works as a part of the interdisciplinary team in the discharge planning of consumers.

Responsibilities
· Conducts nursing assessments and develops care plans for consumers referred for personal care; reassess annually and as needed.
· Participates in the interdisciplinary review of consumer records and provides consultation to care managers and other staff members regarding clinical issues, and community resources.
· Collaborates with care managers to promote cost-effective community service plans.
· Screens consumers for MassHealth Clinical Eligibility.
· Attends meetings as required.
Qualifications:
· Valid Massachusetts RN license; and a Bachelor of Science degree in Nursing or an Associates of Science degree in Nursing and at least one (1) year clinical experience and/or nursing in the community or long-term care setting; or a Diploma RN and two (2) years additional years of community-based nursing experience or long-term experience in community care or long-term care direct service experience.
· Valid driver’s license, travel required.
· Strong communicator, both verbal and written.
· Proficiency in Microsoft Office, Excel, Word; adept at learning new software programs.
· Self-starter, able to prioritize, meet deadlines and manage time effectively.
· Candidate will possess a strong work ethic with a high level of integrity and good interpersonal skills.
· Access to secure internet connection required.
Salary: $71,400.00 per year
Hourly: $39.23 per hour
