Job Summary:

Hertford Rehabilitation and Healthcare Center is seeking a dedicated and compassionate Occupational Therapist (OT) to join our dynamic team. As an OT with our center, you will play a vital role in delivering high-quality, patient-centered care to our residents. Responsibilities:

• Provide comprehensive occupational therapy evaluations and treatments to residents, as ordered by the physician

• Develop and implement individualized treatment plans to promote independence and well-being

• Document patient progress and outcomes in a timely and accurate manner

• Collaborate with the interdisciplinary team to ensure optimal care and treatment

• Participate in facility and rehab meetings, as indicated

• Supervise junior staff members and students, as needed

Requirements:

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy

• SNF experience preferred

• Current license and/or registration in the state (as applicable)
