Responsibilities
- Deliver direct patient care by performing physical examinations, vital signs monitoring, and health assessments with precision and compassion.
- Administer medications, including injections and IV infusions, following strict protocols and safety standards.
- Manage complex care needs such as wound care, catheterization, tube feeding, ventilator management, and pain control.
- Assist with diagnostic evaluations by collecting specimens, processing lab samples, and supporting imaging procedures like sonography or radiology when necessary.
- Document all patient interactions accurately within electronic health record (EHR) systems such as Epic or Cerner while maintaining strict adherence to HIPAA confidentiality policies.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with conditions like dementia, Alzheimer's disease, or developmental disabilities.
- Educate patients and their families on health management strategies including medication administration, discharge planning, and health coaching to promote optimal recovery and independence.
Requirements
- Valid licensure as a Licensed Practical Nurse (LPN) in the appropriate state or jurisdiction.
- Proven experience working in diverse healthcare settings such as long-term care facilities, hospitals (including ICU or trauma centers), hospice care, or home & community care environments.
- Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and clinical documentation standards.
- Hands-on experience with EMR/EHR systems like Epic, Cerner or Athenahealth; proficiency in electronic health records management is highly desirable.
- Ability to perform advanced nursing skills including IV insertion/infusion, catheterization, sterile technique, suturing support, and first aid procedures.
- Excellent communication skills for effective caregiver communication and client education across varied populations including geriatrics, pediatrics or individuals with disabilities.
- Critical thinking skills for triage assessments and acute pain management; familiarity with infection control practices is essential.
- Additional preferred qualifications include experience in emergency medicine (ER), intensive care units (ICU), skilled nursing facilities (SNF), assisted living or memory/cognitive care environments.
