Our patient-centric Primary Care clinics are dispersed across Minneapolis/St. Paul, MN. We are thrilled to serve the Twin Cities metro in our clinics located in Highland Park (St. Paul), Crystal, and South Minneapolis, Rosedale and our newest clinic in Eagan. The Primary Care Physician's Role:
The Primary Care Physician (MD/DO) is a key member of our interdisciplinary care team. In the role, you will evaluate our patients' acute and chronic conditions, manage their multiple medications, mental health, socio-emotional needs, and provide empathetic patient education. The PCP will work closely with fellow clinical teammates to provide comprehensive primary care. While direct patient care is the primary focus of the position, our PCPs also assist with development, implementation and achievement of key organizational objectives and metrics. Functioning as a value-based care model, our team of clinicians proactively and strategically manage our population's health in accordance with IHI's Quintuple Aim, adopt best practices in coding compliance and achieve our HEDIS quality metrics.
