Job Summary
The Registered Nurse is responsible for the care delivery of agency patients, carried out within the scope of care defined by the state laws and the state Board of Nursing. The care delivery, and employee actions, must be within accordance with the federal Conditions of Participations.
Qualifications
- A person who is a registered nurse (RN), has graduated from an approved school of nursing and is licensed in the state where practicing.
- Has an encumbered, active nursing license, within the state that Heroes Home Health serves
- Demonstrates accurate knowledge in case managing the patients within the home health industry.
- Ability to understand and manage the patient episode of care and patient outcomes.
- Must understand and demonstrate ability to complete an OASIS, comprehensive assessment and conduct and carry out day to day intermittent nursing visits.
- Accurate knowledge of collaborating with the physicians and development of the patient's Plan of Care.
Responsibilities
- Directly supervises the assigned patient load, and day to day schedule.
- Provides services that are ordered by the physician as indicated in the plan of care.
- Evaluates patient for home health care services. home health aide, therapy and social work and works with agency personnel to expeditiously assign ordered staff to the patient.
- Works with the patient, legal representative and caregiver in partnership and other disciplines in the development of a comprehensive patient care plan.
- Conducts nursing care within the scope of practice, as well as agency policies and regulations.
- Prepares clinical notes.
- Provides patient and caregiver education.
- Provides patient, caregiver and family counseling.
- Consistently follows agency policies and procedures to set an example for employees.
- Maintains compliance with applicable federal, state, accreditation and local standards, rules and regulations.
- Participates in personal professional growth and development.
- Coordinates with the interdisciplinary team to assure coordination of care and continuity of care delivery.
- Case manages patients effectively and achieves positive patient outcomes.
- Effectively manages the patient episode of care, demonstrating excellence in quality of care and improvement to Heroes Home Health outcomes.
- Communicates with all physicians involved in the plan of care or other health care practitioners related to the current plan of care
- Participates in Heroes Home Health's QAPI program.
- Participates in organization sponsored in-service and educational programs.
Working Environment
Works in agency office space periodically, but mostly works remotely in the patient home environment.
Lifting Requirements
Ability to perform the following tasks if necessary:
- Ability to participate in physical activity.
- Ability to work for extended periods while standing and being involved in physical activity
- Moderate lifting
