Heritage Senior Living is seeking a compassionate and dependable Clinical Supervisor / Licensed Practical Nurse (LPN) to join our clinical leadership team.

This position offers the opportunity to combine hands-on nursing with clinical leadership. You'll work closely with the Director of Nursing to support resident care, oversee caregivers and CNAs, respond to changes in resident condition, and help ensure our residents receive safe, compassionate, and individualized care.

What You’ll Do

Reporting to the Director of Nursing (DON), the Clinical Supervisor / LPN evaluates, plans, coordinates, and provides nursing care within the LPN's applicable scope of practice and in accordance with Heritage Senior Living policies and applicable regulatory requirements.

Provide Resident-Centered Clinical Care

• Provide nursing care based on resident needs, individualized service plans (ISPs), physician orders, and applicable standards of practice.
• Administer medications and treatments in accordance with physician orders and applicable requirements.
• Monitor residents for changes in physical, cognitive, behavioral, or emotional condition.
• Perform appropriate resident evaluations related to changes in condition.
• Make routine checks on residents who are acutely ill or experiencing a change in condition.
• Assist the DON with developing, reviewing, and maintaining individualized service plans.
• Update ISPs as appropriate following changes in resident needs or condition.
• Maintain professional and compassionate relationships with residents and their families.
• Protect resident dignity, privacy, confidentiality, and property.

Coordinate & Follow Up on Resident Care

• Complete appropriate follow-up related to:
  • Changes in resident condition
  • Physician notifications and orders
  • Family/responsible-party notifications
  • Laboratory results
  • Accidents and incidents
  • Skin alterations
  • New admissions
  • Discharges
• Review resident charts, progress notes, incident reports, and other clinical documentation for accuracy and completeness.
• Oversee the 24-hour report and ensure significant resident concerns receive appropriate follow-up.
• Communicate important resident information to the DON and appropriate members of the care team.
• Collaborate with other departments to promote continuity and quality of resident care.

Lead & Support the Care Team

• Provide clinical direction and support to caregivers and CNAs.
• Monitor resident care to ensure services are provided according to the ISP and applicable requirements.
• Review caregiver and CNA documentation for accuracy, completeness, and appropriate follow-through.
• Coach, educate, counsel, and provide performance feedback to care team members as appropriate.
• Assist with addressing employee performance or disciplinary concerns in collaboration with nursing and community leadership.
• Assign and coordinate tasks based on resident needs and staff responsibilities.
• Provide education and training to care team members as needed.
• Promote teamwork, accountability, professionalism, and resident-centered care.

Support Quality & Clinical Compliance

• Maintain knowledge of applicable Wisconsin requirements for CBRF and/or RCAC settings, including DHS 83 and DHS 89 as applicable.
• Practice within the applicable Nurse Practice Act and LPN scope of practice.
• Maintain accurate, timely, and detailed documentation in residents' electronic medical records.
• Assist with reviewing and investigating reportable, sentinel, accident, and incident events as assigned.
• Participate in root-cause review and development of interventions designed to reduce recurrence.
• Help identify clinical trends, resident risks, and opportunities to improve quality of care.
• Maintain high professional and ethical standards and represent Heritage Senior Living positively.

Collaborate With Community Leadership

• Partner closely with the Director of Nursing and other community leaders.
• Participate in daily care-focused meetings.
• Participate in weekly care coordination meetings.
• Attend and participate in leadership meetings as assigned.
• Communicate effectively with nursing, caregivers, families, physicians, pharmacies, and other healthcare partners.
• Assist the DON with clinical operations and other responsibilities as assigned.

Qualifications

• Current and valid Wisconsin Licensed Practical Nurse (LPN) license in good standing.
• Knowledge of professional nursing principles and standards applicable to geriatrics, senior living, and long-term care.
• Knowledge of or ability to learn applicable Wisconsin CBRF and RCAC requirements, including DHS 83 and DHS 89.
• Ability to practice within the applicable Nurse Practice Act and LPN scope of practice.
• Strong clinical judgment, organizational, and problem-solving skills.
• Ability to provide direction, assign responsibilities, encourage staff, and promote positive resident outcomes.
• Strong written and verbal communication skills.
• Ability to function independently while collaborating effectively with the interdisciplinary team.
• Ability to maintain positive relationships with residents and families.
• Ability to manage multiple priorities and adapt to changing resident and operational needs.
• Competency or ability to become competent in the safe use of applicable resident-assistance equipment, including floor lifts, sit-to-stand lifts, and friction-reducing devices.
• Ability to travel as needed.
• Ability to meet applicable pre-employment, training, licensing, and regulatory requirements.

Senior living, assisted living, memory care, CBRF, RCAC, long-term care, or geriatric nursing experience is strongly preferred.
