Heritage Senior Living is seeking an experienced, compassionate, and highly organized Director of Nursing (DON) to lead clinical services and resident care within one of our senior living communities.

Reporting to the Executive Director, the Director of Nursing is a key member of the community leadership team and is responsible for overseeing resident care, clinical documentation, medication management, regulatory compliance, infection prevention, caregiver/CNA development, and clinical quality.

This is an excellent opportunity for a nurse leader who enjoys building relationships with residents and families, developing strong care teams, and creating systems that support exceptional resident outcomes.

 

What You'll Do

Lead Resident Care & Clinical Services

You'll help ensure every resident receives individualized, compassionate care based on their needs and preferences.

  • Complete clinical assessments and required documentation for prospective and new residents
  • Coordinate resident admissions and ensure required medical documentation is completed prior to move-in
  • Develop Individual Service Plans (ISPs) within required timeframes and update them based on scheduled reviews or changes in condition
  • Conduct regular resident wellness visits and monitor changes in health or functional status
  • Coordinate ISP and care-planning meetings with residents, families, and appropriate team members
  • Partner with residents and families to ensure medical and wellness needs are appropriately addressed
  • Track physician appointments and serve as a liaison between residents, families, providers, and community staff
  • Communicate changes in resident condition, medications, treatments, or services to appropriate team members
  • Help ensure dietary, clinical, and other special resident needs are identified and communicated before move-in

 

Oversee Medication Management

Medication safety is an important component of this leadership role.

  • Oversee the community's medication management program
  • Conduct routine medication and documentation audits
  • Monitor Medication Administration Records (MARs) and related clinical documentation
  • Ensure medication orders are accurately received, documented, and communicated
  • Monitor medication availability and help ensure medications are ordered and available when needed
  • Educate and train authorized employees regarding medication administration policies and procedures
  • Monitor medication errors, identify trends, and implement corrective action when appropriate
  • Coordinate periodic reviews of psychotropic medications and resident needs with appropriate healthcare providers
  • Ensure medication and treatment changes are communicated promptly to caregivers/CNAs

 

Develop & Support the Care Team

Great resident care starts with a confident, knowledgeable, and supported team.

  • Provide clinical leadership, supervision, coaching, and education to caregivers/CNAs and other applicable clinical team members
  • Assess employee competencies and identify opportunities for additional training and development
  • Participate in recruiting, interviewing, onboarding, and training care team members
  • Partner with the Executive Director to develop and maintain appropriate staffing schedules
  • Provide ongoing education regarding resident care, medication administration, infection prevention, documentation, and regulatory requirements
  • Address clinical performance concerns and reinforce accountability for established care standards
  • Provide appropriate oversight to LPNs serving in management roles
  • Foster a collaborative, respectful, and resident-centered care environment

 

Maintain Clinical Quality & Compliance

You'll provide clinical leadership to help ensure the community meets applicable regulatory and company standards.

  • Develop and maintain knowledge of applicable federal, Wisconsin, and company regulations, policies, and procedures
  • Ensure clinical practices and documentation meet applicable regulatory requirements
  • Review daily clinical documentation, reports, and resident concerns
  • Monitor and investigate resident incidents and accidents and ensure appropriate follow-up
  • Oversee infection prevention and control practices
  • Monitor infection trends and implement interventions when appropriate
  • Assist with applicable OSHA and workplace safety requirements
  • Conduct routine clinical audits and implement corrective action when opportunities for improvement are identified
  • Maintain accurate, timely, and complete resident records
  • Support the community during regulatory inspections and surveys

 

Partner With Residents, Families & Providers

The Director of Nursing is an important resource for residents and their families throughout their senior living journey.

  • Build positive, trusting relationships with residents and families
  • Communicate resident health concerns and changes in condition promptly and professionally
  • Collaborate with physicians, pharmacies, hospitals, hospice, home health, therapy providers, and other healthcare partners
  • Help residents and families navigate changes in care needs
  • Participate in resident and family meetings as appropriate
  • Serve as a knowledgeable clinical resource for community leadership and employees
  • Assist with prospective resident inquiries and community tours when needed

Qualifications

  • Graduate of an accredited school of nursing
  • Current Wisconsin nursing license in good standing
  • At least one year of nursing experience working with older adults preferred
  • Assisted living, memory care, skilled nursing, or other long-term care experience strongly preferred
  • Previous supervisory or nursing leadership experience preferred
  • Working knowledge of dementia and memory care preferred
  • Strong computer skills, including proficiency with Microsoft Office applications
  • PointClickCare experience is a plus
  • Ability to communicate effectively in English, both verbally and in writing
  • Ability to meet applicable pre-employment and ongoing requirements established by state and federal regulations
