Licensed Practical Nurse - Charge Nurse

Heritage Retirement Community is a licensed personal care home located in Greencastle, Pennsylvania with a family like atmosphere.

Job summary for the position:

Oversee the direct delivery of resident care, administers and oversees prescribed medications, participates in assessments/care planning, communicates with house pharmacy and provider offices, responds to on call duties, etc.

Required work experience, skills or education:

Current and valid Pennsylvania LPN license

Previous supervisory experience a plus however not necessary

Pervious experience working in a senior living setting a plus

Computer skills, excellent communication skills both verbal and written.

Responsibilities

• Provide direct patient care as needed, such as answering call bells and assist with ADLs as needed
• Assist with medication administration as needed
• Responsible for overseeing all orders and completing treatments
• Initiate and conduct thorough patient assessments and monitor vital signs to ensure optimal health outcomes.
• Maintain accurate electronic health records (EHR) while adhering to HIPAA regulations.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Communicate professionally with other health care providers
• Communicate effectively with residents and their families to provide education and support.
• Supervise and mentor junior direct care staff, fostering a collaborative work environment.

Job Type: Part-time

Pay: $30.00 - $32.00 per hour

Expected hours: 16.0 – 24.0 per week

• senior care: 1 year (Preferred)

License/Certification:

• PA LPN license (Required)
