In-House Physical Therapist Assistant

Springfield Nursing and Independent Living in Springfield, OH is seeking a PRN Physical Therapy Assistant to join our growing In-House therapy team. Wooster Health Care Center is a post-hospital rehabilitation and skilled nursing care facility that offers comprehensive Post-Acute Recovery programs in a beautiful, peaceful country setting just minutes from downtown Wooster. Our facility provide some of the area’s highest level of rehabilitation, nursing and medical support to those needing post-hospital care and services.

Essential Duties & Responsibilities:

• Administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing
• Assure all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Monitor patient response to treatment intervention
• Complete required forms and documentation in accordance with company policy and state/federal regulations
• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized fitness-oriented health care programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain and improve mobility.
• Advising patients on exercise techniques.
• Advising patients and their families about in-home treatment options.
• Providing educational information about injury prevention, ergonomics and ways to promote physical health.
• Consulting and collaborating with other healthcare professionals.
• Documenting patient care history.
• Complying with rules, regulations, and procedures.

Springfield Nursing and Independent Living is a 90-bed senior living community offering assisted living and memory care. Set in the "Champion City," the facility sits in a region rich with industrial history and outdoor beauty.:

• Associate degree from an accredited Physical Therapist Assistant program.
• Current PTA license in the state of employment.
• CPR certification required.
• Previous experience in skilled nursing, rehab, or long-term care preferred.
• Strong understanding of rehabilitation principles, restorative care, and geriatric populations.
• Knowledge of Medicare Part A, Part B, and managed care preferred.
• Excellent communication and teamwork skills.
