Overview

Job Title: Licensed Practical Nurse (LPN) — Personal Care Home/Assisted Living

Summary The LPN will assist with managing the daily needs of the residents and and assist Med Tech and PCA staff with comunicating to physicians and obtaining physician orders. Oversee hands-on resident care in a non-acute, home-like setting, focusing on medication administration, health monitoring, and support of residents’ daily living and RASP's. Works collaboratively with the administrator, physicians, Med Techs/PCAs, and families to promote safety, dignity, and quality of life.

Key Responsibilities

• Medication management
• Administer prescribed medications and treatments per provider orders and community policy as needed.
• Maintain accurate eMAR/MAR documentation; monitor for side effects and efficacy.
• Assist and cue residents with self-administration when appropriate; manage controlled-substance counts.
• Health monitoring and reporting
• Obtain and document vital signs, weights, blood glucose, and other routine measures.
• Observe and document changes in condition; promptly report concerns to the RN and/or provider.
• Support RN/provider-directed care plans; participate in service/care plan updates.
• Direct resident care
• Provide basic nursing care (e.g., wound care per orders, catheter care, oxygen setup, simple dressing changes using aseptic technique).
• Assist residents with ADLs when needed in collaboration with CNAs/PCAs; promote independence and fall prevention.
• Respond to emergencies, provide first-line nursing interventions, and coordinate EMS as needed.
• Coordination and communication
• Communicate effectively with residents, families, and the care team; support safe transitions (appointments, labs, home health/hospice coordination).
• Document care timely and completely per policy and regulatory standards.
• Support infection prevention practices and uphold OSHA and community safety policies.
• Compliance and quality
• Maintain regulatory readiness, medication storage standards, and narcotic controls.
• Participate in audits, shift handoffs, in-services, drills, and quality improvement activities.

Qualifications

• Active, unencumbered LPN license in the state of practice.
• Current BLS/CPR certification.
• Prior experience in assisted living, memory care, LTC, or home care preferred.
• Proficiency with eMAR/EHR systems and basic computer skills.
• Strong assessment observation skills, attention to detail, and resident-centered approach.
• Effective communication, teamwork, and de-escalation skills; able to maintain professionalism with residents with dementia.

Physical/Schedule Requirements

• Ability to stand/walk for extended periods; lift/pivot up to 25–50 lbs with assistance.
• Flexible availability (evenings, weekends, holidays) per community needs.

Preferred Competencies

• Dementia/Alzheimer’s care knowledge; Gentle Persuasive Approaches or similar training a plus.
• Medication administration in assisted living settings; familiarity with state AL/PC home regulations.
• Basic wound care, glucometer use, oxygen therapy, and catheter care per policy.

Pay: $28.00 - $33.00 per hour
