What You’ll Do:
As a PRN Health Services Coordinator (LPN or RN), you’ll play a hands-on role in improving the lives of our residents by:
- Being a vital part of our Health Services team by working every other weekend and flexibly covering shifts during vacations, guaranteeing uninterrupted compassionate care for our residents.
- Conducting comprehensive resident assessments prior to move-in, re-entry, and on an ongoing basis to ensure all physical, mental, and social needs are met.
- Assisting with community readiness for regulatory inspections, including maintaining accurate service plans, medication records, and emergency preparedness.
- Establishing and fostering positive relationships with residents, families, associates, home health, hospice, and other external partners.
- Coordinating and ensuring proper health-related training and education for the caregiver team, conducting skill observations to optimize resident care delivery.
- Providing direct resident assistance as needed, including personal care activities and administering medication in accordance with physician orders.
