What are the responsibilities and job description for the Hospice RN position at Heritage Home Health and Hospice?
Heritage Home Health and Hospice is currently looking for a Full time Hospice RN to join our team in Tyler, TX. This position will service Tyler and surrounding areas. Monday through Friday with some rotating call on weekdays and weekends. Pay is competitive.
Essential Functions:
- Ability to administer skilled nursing care for clients of all ages in their place of residence.
- Coordinate care with the interdisciplinary team, patient/family and referring agency.
- Applies specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient’s eligibility.
- Implements/develops/documents the plan of care to ensure quality and continuity of care and recommend revisions to the plan, as necessary.
- Consults with and educate the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs.
- Prioritize any needs with the members of the IDG/T. Provide training to other staff as needed.
- Determines scope and frequency of services needed based on acuity and patient/family needs.
