Overview
Heritage Home Healthcare is seeking a compassionate and dependable PRN Caregiver for a specific client assignment in Northeast Albuquerque (87111).
This assignment includes assistance with intimate personal care. In accordance with the client’s privacy needs, the client has requested a same-gender caregiver for this specific role.
In this position, you will provide delegated personal care services in a home setting, supporting daily living activities while ensuring comfort, safety, and dignity.
Schedule
Monday–Thursday, 11:30 AM–2:00 PM
(Client may prefer a 9:00 AM start time if available)
Care is provided by other staff Friday–Sunday.
Key Responsibilities
- Assist with personal care tasks including bathing, dressing, grooming, toileting, and eating
- Follow prescribed healthcare plans, including assisting with exercises and delegated medication support
- Provide mobility assistance (transfers from bed, chair, or wheelchair)
- Maintain a clean and safe home environment with light housekeeping
- Provide structured support for a client with autism and mental health needs
- Assist with organization, scheduling appointments, and errands
- Escort or transport client to appointments as needed (reliable transportation required)
- Monitor and report changes in client condition to the clinical team
What We’re Looking For
- Compassionate, organized, and dependable caregiver
- Experience supporting individuals with autism and/or mental health conditions preferred
- Strong communication and time-management skills
- Ability to work independently and follow care plans
- Bilingual (English/Spanish) is a plus
- Must have reliable transportation
Qualifications
- At least 18 years of age
- High school diploma or GED preferred
- Previous caregiving experience preferred
- Ability to follow delegation instructions accurately
