This is a hybrid position (2 days remote, 3 days in the field), giving you the flexibility to work from home while building strong, in-person connections with members across Riverside County.
Key Responsibilities
- Serve as the primary point of contact for members and coordinate their care
- Develop and manage personalized care plans with members and providers
- Collaborate with healthcare teams and community resources
- Support members through education, advocacy, and engagement
- Identify and address gaps in care
- Maintain accurate, timely documentation
Requirements
- Experience in care or case management (community-based preferred)
- Knowledge of Medi-Cal and ECM strongly preferred
- Strong communication and interpersonal skills
- Ability to work in a team-oriented environment
- Willingness to travel within Riverside County
- Bilingual (English/Spanish) preferred
