We are seeking a Housing Navigator to join our Enhanced Care Management (ECM) team. This role will help connect individuals experiencing housing insecurity to safe, stable housing solutions while addressing their whole-person health needs.
What You’ll Do:
Work with Medi-Cal members to secure housing and connect them with community resources.
Collaborate with HHN’s multidisciplinary team (RNs, social workers, therapists, and more) to deliver person-centered care.
Build trusting relationships and empower members toward independence and long-term stability.
Requirements
At least 2 years of relevant experience.
Location: Remote after the first month. In-person training is required in Riverside, CA.
Schedule: Full-time, Monday–Friday
Bilingual Required: English/Spanish
