Summary:
Behavioral Healthcare Care Coordinators are licensed social workers trained in managing medical, mental health, and substance use disorder resources. They excel at identifying and connecting patients with appropriate mental health services, support groups, and treatment programs tailored to individual needs. These professionals offer specialized support for patients facing mental health challenges, addiction, and/or co-occurring medical conditions. Their responsibilities include providing crisis intervention and helping patients access the necessary behavioral health and medical services.
In addition to direct care coordination, they serve as the clinic outreach coordinator, identifying and addressing community needs while advocating for underserved populations. Behavioral Healthcare Care Coordinators also implement best practice policies and protocols for Heritage Health, ensuring high standards of care are consistently met.
Requirements
Minimum qualifications:
- Licensed Social Worker (LSW) or higher, required.
- Ability to pass Idaho Department of Health and Welfare Criminal History Unit enhanced background check, required.
- Trained in Mental Health First Aid, required.
- Working knowledge of administrative procedures within a federally funded community health center, preferred.
- Knowledge of Medicaid/Optum/Managed Care regulations.
Your Essential Duties:
- Provide appropriate triage support for individuals identified by medical or mental health providers as needing supportive health or social services.
- Collaborate as a member of the treatment team to provide comprehensive care.
- Offer crisis response and support when required.
- Maintain electronic client files, ensuring completion of assessments and encounter notes.
- Coordinate care by addressing identified needs and connecting clients with appropriate supportive services.
- Ensure compliance with confidentiality policies in accordance with client rights protections.
- Facilitate effective communication with the treatment team to ensure coordinated care.
- Make Child Protection Services (CPS) referrals when necessary.
- Provide all services in a professional and respectful manner at all times.
- Understand and adhere to all Heritage Health/Family Support Services Policies and Procedures.

- Proficient in computer skills, including windows-based software and experience with Electronic Health Records (EHR).

Job Overview
- Working Conditions: Work is normally performed in a typical interior/office work environment in an office building. Work may be demanding and chaotic at times. May be exposed to patient population that will present a variety of contagious diseases, physical injuries, and emotional states of mind.
- Physical/Mental Requirements: Office staff is required to stand, sit, and be mobile one-third to two-thirds of the time. They are required to use hands or fingers to handle or feel over two-thirds of the time, while reaching with hands and arms occurs one-third of the day. Climbing or balancing, stooping, kneeling or crouching occur less than one-third of the time. Communicating by talking/hearing occurs over two-thirds of the day. Amount of time spent lifting or exerting force is about 50% for up to 10 pounds and less than one-third of the time up to 25 – 40 pounds. Rarely is there a need to lift more than 41 pounds. Must be able to manage efficient keyboard use, hear, see, speak and read.
