The Director of Social Services will plan, direct, coordinate psycho-social needs in long term care. Responsible for working with residents in evaluating residents' social needs, easing the transition to the facility, planning and organization of social activities for residents and their families. Recognizing resident needs this may include transfers to appropriate facilities, working with facility team and family members. Assist residents in maintaining quality of life in the most appropriate setting.
Qualifications:
- High School diploma or equivalent
- Minimum 18 years of age
- Prefer college classes in sociology and/or psychology
- Certificate of attendance for accomplishment of 36 hour basic course for social service designee in log term care or assisted living facility.
Salary : $17 - $22
