Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine individual care needs.
- Administer medications, injections, IV infusions, and manage catheterization with precision and adherence to safety protocols.
- Monitor residents’ health status using EMR (Electronic Medical Records) and document all medical procedures accurately and timely in compliance with HIPAA and state healthcare regulations.
- Coordinate discharge planning, case management, and utilization review processes to optimize patient outcomes and resource utilization.
- Manage complex medical conditions including dementia care, behavioral health issues, long-term care needs, and acute pain management within assisted living settings.
- Supervise nursing staff and collaborate with interdisciplinary teams to ensure high standards of patient care; lead team management efforts effectively.
- Provide education on health coaching, medication administration, nutrition, infection control, and safety procedures such as sterile technique and aseptic practices.
Qualifications
- Valid RN license in good standing with current state registration.
- Proven experience working with older adults in assisted living or long-term care environments; familiarity with dementia care and memory support is highly desirable.
- Strong knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and healthcare documentation standards.
- Experience with EMR/EHR systems. Proficiency in medical documentation is essential.
- Critical care experience from ICU or Level I/II trauma centers preferred; background in hospital medicine or emergency medicine is advantageous.
- Demonstrated leadership skills with supervising experience managing nursing teams or care facility staff.
- Skilled in patient assessment techniques including vital signs monitoring, venipuncture, phlebotomy, tube feeding management, ventilator support, and catheter care.
- Knowledge of physical examinations, diagnostic evaluation methods, medication administration protocols including injection techniques and IV insertion.
- Familiarity with healthcare regulations such as HIPAA and long-term care regulations; understanding of discharge planning and case management processes.
- Excellent communication skills for managerial coordination and family interactions; ability to work effectively within multidisciplinary teams.
- Spanish fluency not required, but helpful.
- Pay rate will depend on level of experience.
