Overview
We are seeking a dedicated and compassionate Registered Nurse to join our Home Care team, providing high-quality patient-centered care in a home setting. Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations.
- Administer medications, injections, IV infusions, and perform catheter care following aseptic techniques and HIPAA regulations.
- Manage airway care, ventilator support, tube feeding, ostomy care, and wound management such as suturing.
- Utilize EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks for accurate medical documentation and record keeping.
- Coordinate discharge planning, case management, and collaborate with multidisciplinary teams to ensure continuity of care.
- Provide specialized care for diverse populations including geriatrics, pediatrics, neonatal patients, individuals with developmental disabilities or dementia.
- Perform diagnostic evaluations such as laboratory specimen collection/processing and medical imaging procedures when applicable.
- Educate patients and families on health coaching, medication administration, nutrition, infection control, and disease management strategies.
- Maintain compliance with infection control protocols and infection prevention standards such as CDC guidelines.
- Assist with post-acute care procedures including hospice & palliative medicine support and trauma or critical care management.
- Support patients with complex needs such as acute pain management, behavioral health issues, or chronic conditions like diabetes or cardiovascular disease.
Experience
- Proven experience in various clinical settings including hospital medicine, ICU/CCU/PICU units, emergency room (ER), trauma centers (Level I & II), outpatient clinics, or post-acute care environments.
- Familiarity with specialized procedures such as cardiac catheterization, spinal taps, gastrostomy tube management, dialysis support, ostomy care, or dermal fillers is preferred.
- Experience working with diverse patient populations including children (toddler & neonatal care), seniors (geriatrics & memory care), individuals with disabilities or complex chronic conditions.
- Proficiency in medical terminology and coding systems such as ICD-9/10 and CPT coding is advantageous.
- Skilled in using EMR/EHR systems like Epic or Cerner for documentation; experience with health information management tools is highly desirable.
- Knowledge of Workers' Compensation law and compliance standards related to home healthcare services is a plus.
