GENERAL SUMMARY
Responsible for providing care to patients who receive Heart transplant as an advanced surgical treatment for heart failure. They facilitate the process of heart transplant from patient identification and evaluation through implantation and post-implantation follow up. They also facilitate the listing of potential transplant recipients and coordinating the process when a heart is accepted.
This role manages the transplant process from patient identification and evaluation through approval and listing for transplant. The coordinator facilitates the evaluation process and guides the patient through the required testing. This position requires strong critical thinking skills and the ability to collaborate closely with physicians and the multidisciplinary care team including psychology, social work, financial, nutrition and other services as needed. Additional responsibilities may include participation in quality improvement initiatives, data collection, and education of patients.
EDUCATION/EXPERIENCE REQUIRED:
· Successful completion of HFHS (Parent Operating Group) Pharmacology test.
· Graduate from an accredited school of nursing. Bachelor's Degree preferred.
· Maintains and demonstrates current clinical knowledge in area of specialty.
· Minimum of 2 years clinical nursing experience in an inpatient or outpatient setting.
· Experience in ICU or Operating room is preferred.
· Current RN license in the State of Michigan.
