and psychotherapy to\
adults, children, adolescents and their families in order to improve
psychosocial functioning.\
The therapist is part of a multidisciplinary team that works together
with medical providers to\
offer services through an integrated care model. This is a full or
part-time position working\
onsite Monday through Friday each week.\

Complete required documentation promptly and efficiently, according to

Participate with the HJMC interdisciplinary team to provide mental
health care\
recommendations for patients and/or multi-disciplinary team meetings

agencies.\
Consult with primary care provider, psychiatrist, substance abuse
counselor, care\
coordinator, and other mental health professionals as needed.\
Make appropriate referrals as needed based on clinical judgment,
providing the patient at\
least 2 options for care.\
Maintain professional behavior and relationships with HJMC staff and
referral resources.\
Participate in activities that promote professional development and use
clinical/\
educational research to enhance therapy services.\
Support and follow the policies and procedures of HJMC.\
Identify psychological, emotional or behavioral issues and diagnose
disorders, using\
information obtained from interviews, tests, records, consultations with
primary care\
providers and reference materials.\
Support medication therapy prescribed by PCP.\
Attend staff meetings and other meeting as required.\
Perform general administrative/office duties as required, including but
not limited to\
copying and filing documents, preparing documents, answering phone,
faxing\
information, entering data, maintaining office cleanliness, etc.\
Fill out and maintain client-related paperwork, including federal- and
state-mandated\
forms.\
Provide crisis intervention, emotional support, resources and referrals
as needed.\
Participate in regularly scheduled caseload consultation with the
psychiatric consultant\
and communicate resulting treatment recommendations to the patients
medical provider.\
Screen and assess patients for common mental health and substance abuse
disorders.\
Facilitate patient engagement and follow-up care.\
Utilize registry and tracking systems to collect and collate data and
ensure that all\
administration, case recording, written reports etc. are maintained to
agreed standards.\
Document patient progress and treatment recommendations in EHR and other
required\
systems so as to be shared with medical providers, psychiatric
consultant, and other\
treating providers.\
Facilitate treatment plan changes for patients who are not improving as
expected in\
consultation with the medical provider and the psychiatric consultant
and who may need\
more intensive or more specialized mental health care.\
Develop and complete relapse prevention self-management plan with
patients who have\
achieved their treatment goals and are soon to be discharged from the
caseload.\
Support the mental and physical health care of patients on an assigned
patient caseload.\
Closely coordinate care with the patients medical provider and, when
appropriate, other\
mental health providers.\
Provide or facilitate in clinic or outside referrals to evidence-based
psychosocial\
treatments (e.g. problem-solving treatment or behavioral activation) as
clinically\
