What are the responsibilities and job description for the Home Health Aide position at Hempstead Park Nursing Home?
Job Summary: The Escort is responsible to ensure the safety and comfort of the resident to and from outside appointments, radiological tests, medical procedures. Escort is to ensure that all completed documentation /consult is collected at the end of the appointment and brought to the facility and handed to the nurse.
Job Duties:
a. The Escort will report to the facility half an hour prior to the scheduled pickup. The Escort will bring the resident down to the lobby fifteen minutes before the scheduled pick-up time. He/she will also check with the nurse before they leave the unit, with the resident, so the nurse can make sure the resident has had all necessary medications and give the Escort any additional papers or instructions. The escort to check that the identification armband is in place with the correct name of the resident he/she is assigned to escort. If no armband, report this to the charge nurse/supervisor immediately so that a temporary arm band can be placed on the resident.
b. The escort will ensure that the seat belt is in place and check that the ambulette driver secured the wheelchair safely and applied the seatbelt appropriately to and from the appointment. If the resident is being transported in Taxi, the escort should ensue to make sure the resident has his/her seatbelt on.
c. The escort to sit in the back of the ambulate/Taxi with the resident to and from appointment.
