Job Title: Case Manager – RPHP Program
Program: Gateway Cities Council of Governments (GCCOG) Renter Protection and Homelessness Prevention (RPHP) Financial Services
Position Summary
The Case Manager for the Rental Assistance & Homeless Prevention Program provides client-centered case management services to individuals and families who are experiencing homelessness or are at risk of homelessness in Service Planning Area 7, LCA 1. The Case Manager supports housing stability through assessment, service planning, coordination of financial assistance, and referrals to supportive services in accordance with program regulations, agency policies, and contractual requirements.
Essential Duties and Responsibilities
- Conduct comprehensive intake, eligibility screening, and needs assessments following contractual requirements
- Verify income, housing status, and documentation required for rental assistance eligibility
- Maintain accurate and complete client files in compliance with contract and HYC record-keeping standards
- Develop individualized housing stability and prevention plans in collaboration with program participants
- Provide ongoing case management, including goal setting, progress monitoring, and follow-up
- Assist clients with budgeting, tenant education, and understanding lease obligations
- Maintain professional boundaries and apply trauma-informed, culturally responsive practices
- Coordinate rental, utility, and arrears assistance in accordance with contract allowable costs and limits
- Communicate with landlords, utility providers, and vendors to prevent eviction and housing loss
- Support clients in resolving landlord-tenant issues and accessing mediation when needed
- Enter and maintain accurate client data in Exym and other required data systems
- Ensure timely completion of required forms, certifications, and reports
- Monitor assistance usage to ensure compliance with contract time and funding limits
- Refer clients to community resources, including employment services, benefits, behavioral health, and legal aid
- Collaborate with housing providers, community partners, and coordinated entry systems
- Participate in case conferences, staff meetings, and required trainings
- Assists with conducting street, shelter, and community-based outreach to individuals and families experiencing homelessness or at imminent risk of housing loss
- Other duties as assigned by leadership
Knowledge, Skills and Abilities
- Knowledge of housing resources in Service Planning Area preferred. Resources can include crisis housing and permanent housing throughout Los Angeles County.
- Knowledge of HUD guidelines and experience working with subsidized housing programs such as Section 8, Shelter Plus Care, Rapid/Recovery Re-Housing, and others.
- Current knowledge of best practice models, including Trauma Informed Care, Mental Health Recovery, Harm Reduction, and Critical Time Intervention.
- Understanding of case management practices and ability to accurately and clearly document case notes.
- Ability to accurately enter client data and progress notes in Exym database.
- Possess valid California driver's license and properly registered vehicle.

REQUIRMENTS:
- High School Diploma or GED (required)
- Bachelor's degree in behavioral health and/or a minimum of 2 years of case management and outreach directly tied to homelessness.
- Experience providing case management services to individuals or families experiencing housing instability or homelessness
- Knowledge of HUD homeless assistance programs and fair housing principles
- Strong organizational, communication, and documentation skills
- Possess a valid California driver's license and have access to a properly registered vehicle.
- Ability to pass a background check and criminal clearance
- Ability to successfully pass a TB screening test.
