POSITION TITLE: In-Home Outreach Clinician, Family Preservation Program
Under the direction of the Family Preservation Program Manager, the In Home Outreach Clinician will:
· Complete a thorough and comprehensive intake and assessment with each family to evaluate specific needs and coordinate all systems of care
· Provide one-hour, in-home counseling and clinical case management sessions to families in and around Service Area 7 once per week, at minimum.
· Conduct unannounced visits and child follow-up sessions, as needed, to ensure child safety.
· Arrange and attend the Multidisciplinary Case Planning Committee Meeting for each family in treatment.
· Provide appropriate resources/referrals to families to assist with court mandates and/or assist with each familys needs, as well as follow-up on progress.
· Collaborate with the Department of Children and Family Services Childrens Social Worker to assist in accomplishing case plan goals and outcomes for families.
· Maintain accurate and timely documentation of all job related activities including weekly progress notes, assessments, monthly reports, treatment plans, and other DCFS correspondence.
· Attend weekly individual and group supervision, as well as, individual and team program meetings.
· Conduct one-time Family Preservation Assessment Services with parents involved with DCFS
· Other duties as assigned.
Qualifications:
REQUIREMENTS:
· Master's Degree in social work, counseling, or related field (or in the process of obtaining Masters Degree and approved to begin practicum).
· Current license (or on track to obtaining licensure) from the Board of Behavioral Sciences as an Associate Social Worker, Marriage and Family Therapist, or Professional Clinical Counselor.
· Ability to work a flexible schedule, including some weekends and evenings
· Permanent U.S. residency or citizenship.
· Vehicle for use to conduct in-home sessions
· Valid California Drivers license, current auto insurance, and good driving record.
· Ability to pass background check and criminal clearance.
PREFERRED QUALIFICATIONS:
· Three years experience in conducting individual, group, and family therapy with children and families.
· Bilingual in English and Spanish
· Experience in working in a community based organization.
· Knowledge of Southeast Los Angeles County.
· Trained in facilitating parenting skills groups.
