Role Description
This is a full-time hybrid role based in Tucson, AZ, with the flexibility to work from home. As a Case Manager, you will work closely with consumers and providers to assess their needs, manage their hours and, and coordinate services. You will also monitor the progress of each individual, provide guidance and support, maintain detailed records, and engage with other care providers to ensure optimal service delivery. Communication with providers, members, and their families is a key component of this role, requiring empathy, patience, and dedication.
Qualifications
- Experience in case management, care coordination, or social work
- Excellent communication and interpersonal skills
- Strong organizational and record-keeping skills
- Empathy, patience, and a dedication to improving the lives of consumers
- Proficiency with computer systems and basic digital tools
- Ability to work both independently and in a collaborative team environment
- Experience in healthcare or caregiving is a plus
- Experience with DDD is a plus
