Hours: Available hours Monday and Wednesday 9a-6p
Position Summary:
The Social Worker is an important member of the HHWC Clinical Team. This position will provide guidance and support as a member of the Emotional Trauma department, by offering day-to-day support of individuals and families in overcoming barriers to health and wellness by providing counseling, resources, and advocacy.
Key Responsibilities:
- Provide clinic support and counseling, including conducting psychosocial assessments to identify clients' needs, strengths, and challenges.
- As identified through the patient’s intake assessment, provide individual and family counseling to address mental health concerns, social issues, or life transitions.
- Develop personalized care plans based on individual client and family needs.
- Connect clients with additional community resources, including housing, financial assistance, food, and transportation.
- Maintain a working relationship with external resources within the community to provide additional support for the clients of HHWC.
- Participate in clinic team meetings, working closely with the Clinical Leader to provide an overview of client activities and external resources being used.
- Serve as a liaison between clients and other healthcare providers, both internally and externally, to ensure continuity of care.
Case Management:
- Acting as Case Manager for all clients by maintaining accurate, timely, and confidential documentation of client interactions and progress.
- Monitor outcomes through the review of care plans with clients and adjust plans as needed.
- Collaborate with the interdisciplinary team to ensure comprehensive care for all client needs.
- Assist clients with required paperwork to access healthcare plans through state of Michigan programs for underserved populations and underinsured individuals.
- Therapy, Motivational Interviewing Skills, and other therapeutic modalities as appropriate based on client needs. Be able to utilize and accurately assess a wide range of screening tools, including the GAD, PHQ-9, PCL-5, Columbia Suicide Severity Rating Scale, and other tools as needed.
- Complete Biopsychosocial Assessments, Treatment Plans, Progress Notes, Crisis Intervention Plans, and Safety Plans per client needs.
- Make recommendations for a higher level of care based on psychiatric symptoms, suicidal ideation, or homicidal ideation.
- Follow all organizational protocols for clients in crisis for psychiatric, suicidal ideation, or homicidal ideation.
Qualifications:
- Master’s degree in social work
- Licensed by the state of Michigan or the ability to obtain licensure before the starting date.
- Experience working with underserved or vulnerable populations is highly preferred.
- Cultural competency and sensitivity to diverse populations.
- Demonstrated clinical and case management skills.
- Ability to develop and implement programs while working closely with all team members and external resource representatives.
- Proven leadership skills.
- Demonstrate strong communication and collaboration skills.
