Job Summary
Help Works Inc. is seeking a compassionate and dependable Health Care Coordinator to oversee daily health-related needs and support for six individuals residing in our group homes. This position plays a vital role in coordinating medical care, empowering clients to live fulfilling lives, and ensuring high-quality support through direct supervision and teamwork.
Duties
- Coordinate and schedule all doctor, dental, and specialist appointments for assigned individuals.
- Provide transportation to and from medical appointments as needed.
- Communicate and relay medical information promptly to the Program Coordinator and Nurse.
- Place and track medication orders to ensure timely delivery.
- Deliver necessary paperwork and documentation between the group home and the work shop.
- Respond to work calls and messages within the allotted response time.
- Serve as weekend on-call staff on a rotation basis and be available to fill shifts if coverage cannot be found.
Qualifications
- Prior experience working with individuals with intellectual or developmental disabilities preferred.
- Strong organizational and communication skills.
- Valid driver license and reliable transportation.
- Ability to work independently and as a part of a team.
