Job Summary:The Hospice Chaplain provides and coordinates the delivery of spiritual and emotional support to patients, their families, caregivers, and staff.Job Responsibilities:-Supports and guides patients, their families, caregivers, and staff to assist them through periods of transition, grief, loss, and anxiety.-Provides spiritual interventions at times of crisis or death.-Guides patients’ families and caregivers through spiritual and existential questions and concerns regarding death.-Provides comfort and solace during the end-of-life experience.-The Chaplain must always be appreciative of and respectful toward the full range of individual beliefs, customs, traditions, and practices represented in the community, and serve as a liaison between the hospice and patient/family’s community of faith. -Performs daily visits to patients homes to address their spiritual needs.-Coordinates care with the hospice interdisciplinary team to ensure that all of the patients and family’s needs are met.-Facilitate communication between patients, family, and the patients residential care facility to ensure all rcfe regulatory requirements are met.-Maintains accurate documentation of patient visits.-Available outside normal business hours for emergency response on-call.-Performs other dues assigned.Skills:Required Experience-Experience working with death and dying, individuals/family/caregiver.-Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.-Business acumen.-Customer satisfaction.-Innovation.-Trust & accountability.-Oral/Written Communication.-Patient and Family Sensitivity.-Travel.-Diversity.-Technology & Equipment Skills.-Interpersonal Skills.Preferred Skills & Experience:-Experience in pastoral care and chaplaincy in a healthcare seng.Education:Required Education:-Master of Divinity degree or equivalent from an accredited theological/faith-based institution.-Minimum of two units of Clinical Pastoral Education.-Ecclesiastical endorsement.Preferred Education:-N/ARequired Certifications & Licensure:-Board Certified Chaplain in good standing with a recognized and accredited organization.-California Drivers License.
