- Provide one‑on‑one skilled nursing care to pediatric and adult clients with chronic, complex, or medically fragile conditions in their homes.
- Collaborate with physicians, therapists, case managers, and family members to support the client’s individualized plan of care.
- Deliver continuous hourly care, ensuring safety, comfort, and clinical stability throughout each shift.
- Must have a valid Indiana nursing license in good standing
- A minimum of one year of nursing experience as a Registered Nurse - RN
- Current CPR certification (Free re-certification with Adaptive)
- Good organizational and communication skills
