Responsibilities:
What You’ll Do
- Complete initial patient assessments and ongoing reevaluations
- Develop and update individualized plans of care
- Coordinate services across clinical and support teams
- Provide skilled nursing care in the home
- Educate patients and families
- Complete OASIS documentation and clinical notes
- Supervise and support nursing personnel
- Participate in quality and performance improvement initiatives
You will serve as the clinical leader for your assigned patients, ensuring safe, compliant, and compassionate care.
Qualifications:
What You Bring
- Active Indiana RN license
- Graduate of accredited nursing program, BSN preferred
- At least 1 year of clinical nursing experience
- Home health experience preferred
- Experience with OASIS and ICD 10 documentation preferred
- Ability to work independently with strong clinical judgment
- Passion for community based care

Salary : $72,000 - $77,000
