Job Summary
We are seeking a dedicated Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care to patients in their homes on an intermittent basis. This role operates under the direction and supervision of a Registered Nurse (RN), following physician orders and care plans in line with our Agency’s policies.

- Graduate of a state-approved school of practical (vocational) nursing and currently licensed in the state(s) of practice.
- Minimum of one (1) year of nursing experience preferred.
- Alignment with the philosophy and goals of our Agency.
- Ability to exercise initiative and independent judgment.
- Adhere to established policies and procedures.
- Implement nursing care plans for each patient.
- Provide nursing services, treatments, and diagnostic and preventive procedures as assigned.
- Initiate preventive and rehabilitative nursing procedures as appropriate.
- Observe signs and symptoms, reporting reactions to treatments to the physician and RN.
- Teach and counsel patients and their families regarding nursing care needs and related problems at home.
- Evaluate the effectiveness of the LPN/LVN’s nursing service with the RN.
- Maintain accurate and complete records of observations, treatments, and patient care.
- Participate in medical record audits as assigned.
- Attend staff meetings, patient care conferences, and in-services as scheduled.
- Take on-call duty, including nights, weekends, and holidays as assigned.
- Submit schedule changes to the Director of Clinical Services/Nursing Supervisor daily.
- Participate in patient care conferences for team collaboration.
- Prepare clinical and progress notes.
- Assist physicians and RNs in performing specialized procedures.
- Prepare equipment and materials for treatments.
- Observe aseptic technique as required.
- Assist patients in learning appropriate self-care techniques.
- Work indoors in Agency offices and patient homes.
- Travel to/from patient homes.
- Supervised by: Director of Clinical Services/Nursing Supervisor/RNs
- Ability to participate in physical activity.
- Ability to work for extended periods while standing and being physically active.
- Heavy lifting.
- Extensive bending, lifting, and standing regularly.
