Responsibilities

What You’ll Do:

• Makes the initial evaluation visit to determine patient needs, including type and amount of services needed. - Initiates the patient’s plan of care and service plan.
• Completes comprehensive assessment and receives and sends physician orders.
• Collaborates with Client Intake team to determine readiness of Home Health referrals.
• Collaborates with Recruiting team to ensure timeliness of patient admissions.
• Completes patient discharge visits as required following a patient discharge from Help at Home.
• Responsible for ensuring regulatory compliance with initial plan of care.
• Identifies patient needs and assists in directing them to the appropriate resource.
• Coordinates care with other agencies and providers involved in patient’s care plan.
• Attends marketing opportunities to promote and recruit new Help at Home clients.
• Counsels the patient and family/significant others in meeting nursing and related needs.
• Appropriately utilizes ICD-10 codes.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Performs other related duties as assigned.

Education And Experience

• Graduate of an accredited nursing school. BSN preferred.
• Minimum of two (2) years clinical nursing experience, preferably in home health/home care setting.
• RN licensure and ability to obtain additional states of licensure.
• Experience working with medically fragile clients, addressing medical, social and behavioral components of care.

Physical Requirements

• Sedentary – ability to remain in a stationary position for extended periods of time.
• Ability to move, transport, or position:
• up to 50 pounds
• up to 100 pounds
• Ability to move or traverse about in offices and/or client homes, including ascending and descending stairs.
• Ability to communicate effectively and clearly with others to exchange information.
  

Travel Requirements

• Regular travel on a daily or weekly basis required, even in inclement weather

Pay Range

USD $75,000.00 - USD $77,000.00 /Yr.
