What are the responsibilities and job description for the RN Case Manager Hospice position at Helios Care?
SUMMARY OF POSITION:
As compassionate professionals, the RN Case Manager develops and oversees the plan of care for patients dealing with a serious, life-limiting illness, working closely with the patient’s provider and the hospice interdisciplinary team. To promote comfort and quality of life the RN provides skilled nursing care; guidance and education to families and caregivers on recognizing and managing potential symptoms; what to expect; while providing emotional and practical support for both the patient and the family/caregiver. Helios Care RN Case Managers have one-on-one time providing exceptional care and attention to the patients, brightening their days and making a lasting positive impact.
MINIMUM REQUIREMENTS
- Graduate of an accredited school of nursing.
- One to two years of recent acute care experience in an institutional setting, or previous homecare experience preferred.
- Current professional licensure in New York State, and valid driver’s license with driving record
- Bachelor’s degree. Registered nurses with less than a Bachelors degree will be considered based on previous experience.
- Management experience not required but supervisory skills preferred.
- Excellent observation, verbal and written communication skills, problem solving skills,
basic math skills, nursing skills per competency checklist.
Job Type: Full-time
Pay: $77,500.00 - $80,500.00 per year
