To support the Mission of Helio Health, Inc., the CTI Licensed Practical Nurse will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a "critical time" transitional period in their lives.
CTI Licensed Practical Nurse:
CTI is designed specifically to enhance the continuity of care during transition from hospital to community care. Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Critical Time Intervention Licensed Practical Nurse is a care manager, but in addition to Care Management tasks, they resolve medical and rehabilitative needs, when within scope of practice, that impact the individual's ability to meet recovery goals. The LPN will work on a Full-Time basis and using a hybrid work model, working in the community throughout Oneida County.
The schedule is Monday-Friday, 8:00am-4:30pm, with one late shift per week (12pm-8:30pm), and some evening on call responsibilities.
Pay Rate: $56,000 per year.

- Assess basic health and medical needs within their scope of practice.
- Coordinate health care with community medical providers.
- Provide rehabilitation, education, and support services.
- Works to create relationships with key providers in the community.
- Other duties as assigned by supervisor.
- Must be a licensed practical nurse currently registered by the New York State Education Department.
- Experience and/or training in the treatment of severe mental illness and substance use disorders preferred.
- Basic computer skills required.
- Must have a valid NYS driver's license.

Salary and hourly compensation ranges are provided in accordance with NYS law and are based on Helio Health's good faith belief of what is accurate at the time of posting. Salary and hourly compensation offers are based on candidate's education level and experience relevant to the position and also take into account information provided by the hiring manager and program.
