Job Description: Position Title: Nursing Assistant II Position code: NS18_P(cost center) Job Family: Sup II FLSA: Non-exempt Location: Roslindale Title of Immediate Supervisor: Director of Nursing I. Position Summary The primary role of the Nursing Assistant is to protect, sustain, and nurture the residents/patients that live at the Center. They provide assistance with activities of daily living, care of the resident/patient’s environment, and meet other needs as required. The goal of the work of the nursing assistant is to provide assistance that promotes a high quality of life for the residents/patients. The nursing assistant works as a member of a team that seeks to achieve a home like setting while maintaining excellence in all areas of care. In addition, the nursing assistant will demonstrate a strong commitment to the philosophy and mission of Hebrew SeniorLife and recognize resident/patient’s dignity and choice in all aspects of daily life. They are an active participant in the activities, work, and social environment of the unit/households, and strive to make every resident/patient encounter into a positive and meaningful experience and opportunity, while ensuring the provision of safe and efficient quality care. II. Position Responsibilities: 1. Recognize and respond to the needs of the residents/patients and families 2. Recognize and communicate changes in the resident/patient’s condition to the appropriate nursing team member 3. *Adhere to Safe Patient Handling practices, i.e. maintain a no manual lift environment 4. Participate in Quality Improvement programs 5. *Assist residents/patients with all aspects of personal care as indicated in the care plans 6. *Document pertinent information regarding care delivered in the designated areas of the resident/patient’s electronic medical record. 7. Report accidents and incidents as they occur 8. Report any observed or suspected resident/patient abuse immediately 9. Observe and report skin integrity 10. Participate in daily report team and care planning meetings as needed 11. *Be proficient in obtaining vital signs and other procedures such as temperature, pulse, respirations, blood pressure, weight, blood glucose levels, pulse oximetry and urine and stool specimens as directed 12. Perform electrocardiograms and bladder scans 13. Follow infection control practices 14. Attend and participate in ongoing training and educational classes 15. *Maintain confidentiality and privacy of the resident/patient according to HIPPA regulations 16. *Communicate effectively with peers, other team members, residents/patients and families 17. Facilitate life enrichment activities that encourage resident/patient engagement 18. Maintain cleanliness of the resident/patient’s room and common program areas 19. Prepare, serve, and clean up after meals and snacks 20. Participate in committee meetings 21. Other duties as assigned IV. Qualifications: RSU Nursing Assistants: MA Certified Nursing Assistant certification is required in good standing upon hire and on-going. Fluent English skills required – verbal and written Computer literacy required Flexibility and ability to work independently and collaboratively Must complete mandatory HSL/HRC educational requirements and programs Core care competencies Pass nursing assistant skills test with 70% or greater V.
