We are looking for an LVN in the Brownsville area.
GENERAL DESCRIPTION
Home health LVN or otherwise known as licensed vocational nurses are part of health professionals who are responsible in providing
health interventions within a home-based setting. Their primary function is to administer skilled nursing care, under the supervision of a
registered nurse, for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and
referring agency. Communications and documents clinical findings as per agency policy and procedures. Functions with the scope of
practice for LVN and individual competencies.
WORK EXPERIENCE REQUIREMENTS
Minimum of 1 year experience in an providing nursing care in the home health care environment .
Knowledge of the current Texas Board of Nursing Rules and Regulations
Excellent organization, multi-tasking skills, and verbal /written communication skills.
Proficiency in Microsoft Office software
EDUCATION REQUIREMENTS
- Licensed Vocational Nurse (LVN) licensed in the state of Texas
- One (1) year experience providing comprehensive skilled nursing care in the home health environment preferred.
- Individuals must pass the National Council Licensure Exam for practical nurses (NCLEX-PN)
JOB RESPONSIBILITIES AND DUTIES
At all times, employee is responsible for:
1. Protecting the physician’s license;
2. Maintaining confidential any and all information accessible while performing job duties;
3. Staying in compliance with Medicare, Medicaid, OSHA, HIPAA, OIG, and any other legal and/or contractual obligations.
Duties of the Job:
Furnishes skilled nursing services as assigned and in accordance the plan of care..
Prepares clinical and progress notes and submits to the agency according to agency protocols.
Accurately and effectively documents skilled observations and assessments, treatments and medications administered,
patent/caregiver teaching, patient response, and communications with physician and RN of findings follow-up as appropriate.
Reports changes in a patient status to RN and physician.
Updates the medication list with changes.
Documents physician orders and submits to RN for review and implementation of the change in plan of care.
Assists the physicians and RN in performing specialized procedures in accordance with the plan of care, LVN scope of practice,
