- Facilitating clients' involvement in recreational and social activities.
- Providing transportation to community events, appointments, and activities.
- Documenting clients' progress and reporting any concerns to the appropriate parties.
- Collaborating with clients' families, caregivers, and other support professionals to ensure comprehensive care.
