Full-Time Clinical Support Nurse (RN/LPN) – Mobile Wound Care

Heartland Care Advanced Wound Solutions is seeking a full-time Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our growing mobile wound care team throughout the Des Moines and surrounding areas.

This role is ideal for a nurse who enjoys both hands-on patient care and highly detail-oriented operational workflows. The Clinical Support Nurse will perform independent nursing visits, assist with wound care procedures and care coordination, support telehealth-based provider collaboration, and assist with administrative and clinical workflow responsibilities across home, assisted living, skilled nursing, and long-term care settings.

No prior wound care certification is required – we are willing to train the right candidate.

Primary Responsibilities:

• Perform independent nursing visits within home and facility-based settings
• Assist with wound care procedures, dressing changes, patient assessments, and ongoing wound management under provider oversight
• Collaborate with provider through telehealth workflows during designated patient visits and procedures as needed
• Perform clinical intake functions including vitals, medication reconciliation, and patient updates during visits
• Support real-time EHR documentation, wound measurements, imaging, and clinical data entry
• Photograph wounds and upload images into the EHR in real time
• Support clinical documentation workflows and care coordination activities
• Coordinate communication with facilities, pharmacies, providers, home health agencies, and caregivers
• Fax progress notes, orders, and clinical documentation as needed
• Enter patient demographics, pharmacies, provider information, and facility information into the system
• Document patient/facility communications and care coordination activities
• Support telehealth coordination and additional operational workflows as needed
• Perform additional delegated clinical and administrative responsibilities within scope of practice

Position Requirements:

• Active Iowa RN or LPN license in good standing
• Minimum 1 year nursing experience required
• Current CPR/BLS certification required
• ACLS certification preferred
• Current TB testing and immunization records
• Valid driver’s license and car insurance
• Strong attention to detail and organizational skills
• Ability to multitask in a fast-paced mobile healthcare environment
• Comfortable with technology, tablets/iPads, EHR documentation, and mobile workflows
• Strong observational, communication, and critical-thinking skills
• Ability to function effectively within a highly collaborative provider-support workflow
• Ability to maintain professionalism and composure in complex home and facility environments

• This is a mobile healthcare role involving daily travel to patient homes and healthcare facilities
• Reliable punctuality, flexibility, and adaptability are important for success in this role
• Wound care experience is welcomed but not required
• Training will be provided for wound care workflows, procedures, telehealth collaboration, and company-specific processes

LPN: $34-$36/hour depending on experience and qualifications

RN: $38-$40/hour depending on experience and qualifications

• Full-time position Monday through Friday, 9:00 AM-5:00 PM, though occasional flexibility may be required based upon patient scheduling and operational needs
