Position Summary
The In-Home Coordinator is responsible for coordinating in-home care services for participants, maintaining staff schedules, ensuring quality service delivery, conducting participant assessments and reassessments, and supporting caregivers and participants to promote independent living.
This position requires strong organizational skills, attention to detail, problem-solving abilities, and the ability to work independently while managing multiple priorities.
Essential Duties and Responsibilities
- Coordinate and maintain schedules for in-home care participants and aides.
- Match participants with qualified caregivers based on needs and availability.
- Conduct participant assessments, reassessments, and home visits as required.
- Monitor service delivery and participant satisfaction.
- Maintain accurate participant and employee records.
- Recruit, interview, and assist with onboarding new caregivers.
- Provide support and guidance to aides and participants.
- Assist with caregiver training and orientation.
- Respond to scheduling changes, call-ins, and participant concerns.
- Ensure compliance with agency policies and state regulations.
- Participate in after-hours on-call rotation as assigned.
- Prepare reports and documentation as required.
- Perform other duties as assigned.
Pay: $35,000.00 - $45,000.00 per year
