Wellness Coordinator - Southwest OH
Position Summary
Work in coordination with the Wellness Manager to educate and manage the Kroger Cincinnati, OH participants. The coordinator will work daily in stores to promote the wellness program, engage the participants, and increase participation and health care education. The coordinator would also work with the wellness manager and Kroger pharmacy team to schedule on-site screenings for membership and yearly health fairs. This position requires daily/weekly travel to Kroger stores in the Southwest OH area to communicate with members, and occasional travel to the Heartland Fund to pick up promotional material, meet with staff, and plan for future events.
Reports to: Wellness Manager
ESSENTIAL FUNCTIONS:
· Promote the Plan’s wellness and education programs to encourage healthy lifestyles and health awareness, including preventive health, nutrition, and physical fitness activities.
Coordinate with Kroger Ohio to maximize program outreach.
Assist in developing wellness content for quarterly newsletter and wellness communication pieces.
· Provide high quality customer service to participants or potential participants.
· Maintain a positive attitude, cooperating with co-workers, the plan, the union, and participating employers.
· Compose and contribute a story or blog with supporting photo(s) from store gatherings monthly.
· In conjunction with the Wellness Manager and Communication Director, develop and coordinate the yearly health fairs for assigned group.
· Active involvement in the open enrollment process in assigned stores.
· Keep inventory of wellness promotional items for assigned group and track what is given to participants.
· Create and follow the wellness program calendar for your assigned group.
· Maintain relations with store member, union representatives, and Kroger staff.
· Plan and manage yearly on-site screenings for Kroger.
· Annually report program engagement and effectiveness of the Kroger wellness program to the Wellness Manager and Board of Trustees.
· Present at Union and Employer events to educate/promote the Wellness programs.
· Daily store engagement with participants encouraging preventative exams.
ADDITIONAL DUTIES AND RESPONSIBILITIES:
· Assist with training other employees as assigned.
· Assist with special projects as needed and/or other duties as assigned.

· Commitment- willing to do what is necessary and goes above and beyond the call of duty.
Weekly store engagement
· Store engagement throughout the week consists of 25 hours minimum depending on the season.
· Administrative hours shall make up the remainder of the day.
· A weekly schedule should typically consist of 4 days visiting stores, along with an administrative day remote at homework location.
Photo/Experience
· Coordinator(s)should work to collect two photos per month and a general participant interest story (ex. Blog, health story, or monthly educational pieces).
Email/Phone Etiquette
· All phone calls/emails need to be returned within 24 hours of receipt.
· During daily hours, internal phone calls/emails should be responded to promptly.
PROCESSES AND PROCEDURES:
· Educate participants and providers.
· Correspond with and answer questions from participants and providers regarding benefits.
· Embrace new systems and processes to increase efficiency.
FUND RULES AND PROCEDURES:
· Demonstrate and follow Heartland LLC Employee Reference Guide.
· Perform all other duties as assigned.
EXPERIENCE AND TRAINING:
· High School Diploma or GED equivalent- required.
· Bachelor’s degree in health education- required.
· Knowledge of Microsoft Office Suite- required.
· Knowledge of the structure of public health coaching.
· knowledge of health education and cultural wellness.
· Knowledge of simplistic medical testing and medical data analysis.
· Administrative experience including filing, scanning, copying, computer knowledge.
SKILLS AND ABILITIES:
· Thorough knowledge of Heartland plan, provisions, policies, and practices
· Ability to understand verbal and written communication.
· Basic communication skills- required.
· Typing proficiency -required
· Medical Data Analysis and Aggregate Reporting.
· Problem solving skills.
PHYSICAL DEMANDS AND WORKING CONDITIONS:
· Ability to be in a Sedentary position for an extended period of time.
· Ability to bend, stop, and stand as required.
· Ability to lift to 10lbs.
