What are the responsibilities and job description for the Shelter Care Plus Liaison position at Heartland Center For Behavioral Change?
JOB SUMMARY:
The Shelter Plus Care (SPC) Liaison provides a variety of direct and indirect services to individuals referred to and/or receiving Shelter Plus Care Vouchers through the Missouri Department of Mental Health. The Housing Case Manager will practice Housing First model to decrease barriers and improve outcomes for SPC participants.
ESSENTIAL JOB DUTIES, EXAMPLES OF WORK:
Work may include, but is not limited to, the following examples:
1. Attend and be trained to conduct current community selected assessment required for housing vulnerability index scale.
2. Outreach individuals that may qualify and are not engaged with community mental health and/or substance use services. Participate in CORE (Community Outreach Resource and Engagement) meeting and utilize the Show the Way mobile outreach app. Outreach and support in home of participants monthly.
3. Refer and transport (as needed) individuals seeking housing and participate in appointments to establish and maintain services.
4. Act as an advocate for individuals seeking housing using a progressive engagement model.
5. Directly contact potential landlords and provide information on the voucher programs to foster connections and professional relationships and to ensure expeditious connection to fair and affordable housing. Continual negotiations with landlords and vendors.
6. Complete SOAR certification and assist individuals with application for Social Security benefits.
7. Collaborate with community partners and Department of Mental Health to ensure the best outcomes for housing voucher recipients using a trauma informed, equitable, strengths-based approach. All business will be service focused.
8. Refer voucher recipients to support services for employment, housing readiness, mental health, substance use, and other benefits they may qualify to receive.
9. Assist individuals with determining their readiness for permanent housing and provide support with the move in process as needed. Support in home monthly.
10. Work independently to manage priorities and meet all deadlines.
11. Provide data to support future housing initiatives.
12. Participate in Performance Improvement activities related to Swope Health Quality Initiatives.
13. Perform other duties as assigned.

- Ability to work with landlords, transitional housing resources, recovery support providers, employers, and consumers.
- Ability to communicate effectively both orally and in writing
- Skill in placing consumers in housing and employment

EXPERIENCE AND EDUCATION:
The following education and experience qualifications are the minimum requirements used to admit or reject applicants for consideration.
- Bachelor’s degree in human services or related field
OR
- Four years of relevant case management experience in housing or community based mental health services.
- Experience with Shelter Plus Care and similar Housing Assistance Programs is preferred.
Additional Requirements:
- 1-2 years’ experience with individuals receiving psychiatric and/or substance use treatment services.
- Knowledge of Department of Mental Health and HUD housing program requirements and guidelines.
- Valid Driver’s License and reliable transportation
- Ability to write clearly and distinctly.
- Knowledge of community resources.
- Proficient with use of electronic health records, Microsoft Office products including Excel, Word, Teams, Outlook, and PowerPoint.
NECESSARY SPECIAL QUALIFICATIONS:
- All candidates will be required to submit to a background screening. No candidate may be considered for employment who is under the supervision of Federal, State, County or Municipal Corrections Department nor have any pending felony or misdemeanor charges other than minor traffic charges.
- Driving may be required for this position. The successful candidate must have and remain in possession of a valid current driver’s license, automobile insurance, and be insurable under the agency’s motor vehicle insurance policy.
- All candidates for employment must meet the established minimum qualifications for employment as identified by the applicable regulatory agencies.
