Hearth is a non-profit organization dedicated to the elimination of homelessness among the elderly. This mission is accomplished through a unique blend of prevention, placement, and housing programs all designed to help elders find and succeed in homes of their own. To this end, all housing operated by Hearth provides a creative array of supportive services that assist residents to age with dignity, regardless of their special medical, mental health, or social needs. Available Shift: This is a temporary position beginning May 15, 2026, and ending June 19, 2026 (approximately 6 weeks). There may be an opportunity for this role to transition into a per diem position
Schedule: Monday through Friday, 8:00 AM to 2:00 PM (6 hours per day), totaling 30 hours per week.
Pay Rate: $17.50 per hour
Pay Transparency:
This is the fixed hourly rate for this position if hired to work in Massachusetts. The compensation is not part of a range and is set based on the requirements of the role.
Major Functions:
1. Deliver personal care services in line with approved service plans.
2. Provide household support to promote safe, sanitary, and independent living.
3. Observe, record, and report all changes in resident condition.
4. Encourage residents to maintain independence in daily activities.
5. Complete daily logs (EVV & GAFC Logs) and communicate promptly with supervisors.
Duties and Responsibilities:
Personal Care Services
· Assist residents with activities of daily living (ADLs), including bathing, grooming, oral hygiene, dressing, toileting, and meal preparation.
· May check in with residents with daily reminders in regards to food, medication, housekeeping
· Apply universal precautions and infection control in all tasks.
· Recognize and report signs of illness, discomfort, or injury immediately to the nurse care manager.
Household Services
· Complete household tasks such as shopping, laundry, bed-making, dusting, vacuuming, mopping dishwashing, and sanitizing kitchens and bathrooms.
Observing/Recording/Reporting
· Document services and observations in daily logs (EVV & GAFC Logs).
· Report emergencies, behavioral changes, or hazards directly to the nurse care manager.
· Communicate with Nurse Care Manager or Clinical Care Manager upon requests regarding care adjustments.
Therapeutic Relationship
· Approach residents with compassion and respect, maintaining professional boundaries.
· Encourage self-care whenever possible.
· Follow service plan interventions consistently and communicate empathetically.
Education, Experience and Licensure
· High School Diploma required; at least one year of elder care or disability care experience preferred.
· CPR/First Aid/AED certification required within 90 days of hire and must be maintained.
· Complete and Earn MA Certification in Home Health Aide or Personal Care Assistant (PCA). Must maintain certification in good standing.
Required Skills, Abilities and Knowledge
· Ability to read, write, and communicate clearly in English
· Strong computer literacy required — must be comfortable using Outlook, electronic visit verification (EVV) systems, and care management software.
· Strong interpersonal and cultural sensitivity skills.
· Capacity to work independently while collaborating with interdisciplinary teams.
· Observation and critical thinking skills to detect changes in tenant health.
Physical Requirements
· Ability to push/pull/lift up to 35 lbs.
· Frequent standing, walking, stair climbing, and bending.
· Ability to respond quickly in emergencies.
